MoveHowUWant Waiver and Release

You, the participant, agree that when you are participating in the physical portion of the group training, video library, and/or personal training, you do so entirely at your own risk.

This waiver and release of liability includes, without limitation, all injuries which may occur, regardless of negligence, as a result of; 

(a) your participation in any activity, personal training or instruction 

(b) our instruction, training, or supervision

You know that you may suffer physical injury in connection with the activities associated with the video library, and/or personal training. 

You acknowledge that you have carefully read this “waiver and release” and fully understand that it is a release of liability. You expressly agree to release and discharge MoveHowUWant and Shandril Mintenko, and all affiliates, employees, agents, representatives, successors, or assigns, from any and all claims or causes of action and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against MoveHowUWant and Shandril Mintenko for negligence, personal injury, or property damage.

If you are less than the legal age required for consent or require the assent of a care provider, your parent, guardian or care provider must also sign this form.
